It is a virus that attacks the body’s immune system, particularly CD4 T cells, which help protect the body against infections and certain diseases.

AIDS stands for acquired immunodeficiency syndrome. It describes the most advanced stage of HIV infection, when the immune system has become severely weakened.

In simple terms:

HIV is the virus. AIDS is an advanced stage of the disease caused by HIV.

A person can have HIV without having AIDS. With early and effective antiretroviral therapy (ART), many people living with HIV can maintain viral suppression and never progress to AIDS.

AIDS itself is not a separate virus that can be transmitted from one person to another. A person acquires HIV, and untreated HIV can eventually progress to AIDS.

<div id="ez-toc-container" class="ez-toc-v2_0_86 counter-hierarchy ez-toc-counter ez-toc-grey Having AIDS?

Yes. Most people diagnosed with HIV do not automatically have AIDS.

HIV infection can progress through different stages. During the earlier stages, a person may have symptoms, mild symptoms or no noticeable symptoms at all.

If HIV is treated effectively, the virus can be suppressed and the immune system can be protected. As a result, many people living with HIV never develop AIDS.

AIDS generally occurs when HIV has caused substantial damage to the immune system or when certain serious opportunistic infections or cancers develop.

Can AIDS Be Prevented?

In many cases, yes. The main way to prevent HIV from progressing to AIDS is to diagnose HIV early and start effective treatment.

Antiretroviral therapy suppresses HIV replication, reducing the amount of virus in the body and helping preserve or restore immune function. People who start effective treatment before severe immune damage occurs have a much lower risk of progressing to advanced HIV disease.

This is one reason HIV testing and early treatment are important even when a person feels completely healthy.

What Is HIV?

HIV is a virus that targets the immune system.

In particular, it infects and damages CD4 T cells.

These cells play an important role in coordinating the body’s immune response.

As HIV reproduces, it can gradually reduce the number and function of CD4 cells.

Without treatment, continued HIV replication can weaken the immune system and increase the risk of serious infections and certain cancers.

Is HIV a Disease or a Virus?

HIV is a virus.

The term “HIV infection” describes the condition caused by the virus.

The term “AIDS” is used for advanced HIV infection when specific criteria indicating severe immune damage or serious HIV-related illness are met.

This distinction is important because having HIV does not automatically mean that someone has AIDS.

What Is AIDS?

AIDS stands for acquired immunodeficiency syndrome.

It is the most advanced stage of HIV infection.

In Singapore, an AIDS diagnosis can be made when:

The CD4 count falls below 200 cells/mm³, or

The person develops one or more specified opportunistic infections, regardless of their CD4 count.

AIDS therefore reflects significant immune system damage or the development of serious illnesses associated with advanced HIV.

How Does HIV Progress to AIDS?

Untreated HIV can progressively damage CD4 cells.

The general progression can be described as:

HIV infection → chronic HIV infection → severe immune suppression or AIDS

The speed of progression varies.

Some untreated people may develop advanced disease within a few years, while others may remain relatively well for longer.

Effective ART can interrupt this progression by suppressing HIV replication.

What Are the Stages of HIV Infection?

<p high.

Some people develop flu-like symptoms, while others have no noticeable symptoms.

Possible symptoms include:

Fever

Sore throat

Skin rash

Swollen lymph class="wp-block-paragraph">These symptoms are not specific to HIV and can occur with many other infections.

Some people do not experience symptoms at all.

Stage 2: Chronic HIV Infection

After the acute stage, HIV can enter a prolonged stage in which the person may have few or no symptoms.

HIV is still present and can continue reproducing.

Without treatment, this stage can eventually lead to increasing immune damage.

With effective ART, however, HIV can be “cures AIDS”.

Treatment for a person with AIDS generally involves:

ART to suppress HIV

Treatment of existing opportunistic infections

Prevention of additional infections where indicated

Management of complications

Monitoring immune recovery

The overall aim is to control HIV and improve immune function while treating the conditions associated with advanced disease.

Can Someone With HIV Live a Long Life?

Yes.

Early diagnosis and effective treatment have significantly changed the outlook for people living with HIV.

When treatment is taken consistently and HIV remains suppressed, many people can maintain their health and live long lives.

Long-term HIV care may also involve monitoring for other health conditions, including cardiovascular, kidney, liver and metabolic problems.

Can Someone With AIDS Live a Long Life?

Treatment can substantially improve outcomes even when HIV is diagnosed at an advanced stage.

Starting ART can suppress HIV and allow immune function to improve.

However, the outlook depends on factors such as:

How advanced the disease is

The presence of opportunistic infections

CD4 count

Viral load

Response to ART

Other medical conditions

Treatment adherence

Prompt medical care is therefore important after an advanced HIV diagnosis.

What Happens to the Viral Load After Starting HIV Treatment?

The viral load generally decreases after effective ART is started.

The speed of reduction varies between individuals.

Over time, the viral load may become undetectable, meaning it is below the detection limit of the laboratory test being used.

An undetectable result does not mean HIV has been eliminated.

Treatment still needs to continue.

What Does Undetectable Mean for HIV?

An undetectable viral load means that HIV is present at a level below the detection limit of the test.

It does not mean:

HIV has been cured

HIV has disappeared from the body

ART can be stopped

Follow-up is no longer needed

For sexual transmission, however, sustained viral suppression has an important prevention benefit.

A person living with HIV who maintains an undetectable viral load through effective treatment does not sexually transmit HIV.

Can HIV Progress to AIDS Even While Taking Treatment?

Effective ART greatly reduces the risk of progression to AIDS.

However, treatment may not work as intended in every situation.

Possible reasons for poor viral suppression include:

Missed medication doses

Drug resistance

Drug interactions

Problems absorbing medication

An ineffective treatment regimen

Regular viral load monitoring helps identify treatment problems.

If HIV remains detectable despite treatment, the healthcare team can investigate the cause and modify the treatment where necessary.

Can You Stop HIV Treatment If Your Viral Load Is Undetectable?

No.

An undetectable viral load does not mean that HIV has been cured.

If ART is stopped, HIV can usually begin reproducing again and the viral load can rise.

Stopping treatment can also increase the risk of immune system damage and, in some circumstances, drug resistance.

Anyone experiencing difficulties with HIV medication should discuss them with their healthcare provider rather than stopping treatment independently.

Can HIV Treatment Prevent AIDS If Started Early?

Yes.

Starting ART early can help prevent the immune system from becoming severely damaged.

This is one of the main reasons people diagnosed with HIV are advised to begin treatment as soon as clinically appropriate.

Early treatment can:

Suppress HIV

Preserve CD4 cells

Reduce HIV-related illness

Reduce the risk of opportunistic infections

Prevent progression to AIDS

Reduce sexual transmission when viral suppression is maintained

How Can You Tell If HIV Has Progressed to AIDS?

You cannot determine this from symptoms alone.

A healthcare provider may assess:

CD4 count

Viral load

Medical history

Opportunistic infections

Other HIV-related conditions

In Singapore, a CD4 count below 200 cells/mm³ or certain opportunistic infections can meet the criteria for AIDS.

Someone with HIV should therefore attend regular medical follow-up even if they feel well.

When Should You Get Tested for HIV?

You should consider HIV testing if you:

Have had condomless sex

Have multiple or casual sexual partners

Have a new sexual partner whose HIV status is unknown

Have been diagnosed with an STI

Have a sexual partner living with HIV

Are taking PrEP

Have had a potential exposure through contaminated needles

Have had another recognised exposure

Are pregnant and require antenatal screening

Singapore recommends HIV testing at least once for adults aged 21 and above or those who are sexually active, with more frequent testing recommended for people with ongoing exposure risks.

What If You Think You Were Recently Exposed to HIV?

Do not wait for symptoms.

If the possible exposure occurred within the previous 72 hours, seek urgent medical advice about post-exposure prophylaxis (PEP).

PEP involves taking HIV medication after a potential exposure to reduce the likelihood of infection.

It is most effective when started as soon as possible and must be started within the recommended 72-hour window.

Testing may also be needed, but a test performed immediately after exposure cannot rule out an infection acquired during that encounter.

HIV and AIDS Treatment in Singapore

HIV treatment is available through public hospitals and private healthcare services (e.g. Dr Tan Medical Center) in Singapore.

The current national approach recommends ART for people living with HIV, with treatment aimed at achieving and maintaining viral suppression.

People who are diagnosed early and take effective treatment can often prevent progression to AIDS and maintain long-term health.

Singapore also provides medication subsidies for eligible residents, although eligibility and coverage depend on the specific medicine and individual circumstances.

People diagnosed with advanced HIV may require additional care for opportunistic infections and other complications.

The key difference between HIV and AIDS is that HIV is the virus, while AIDS is the most advanced stage of HIV infection.

HIV attacks the immune system, particularly CD4 cells. Without effective treatment, ongoing HIV replication can gradually weaken the immune system and eventually lead to AIDS. In Singapore, AIDS is diagnosed when a person’s CD4 count falls below 200 cells/mm³ or when they develop one or more specified opportunistic infections, regardless of their CD4 count.

Having HIV does not mean that a person will necessarily develop AIDS. Modern antiretroviral therapy can suppress HIV replication, protect the immune system and greatly reduce the risk of progression to advanced disease. People who start effective treatment early may never develop AIDS.

HIV can also remain without noticeable symptoms for a long time. This means that someone can have HIV without knowing it and potentially transmit the virus. Symptoms alone cannot confirm or rule out HIV, so testing is important after a potential exposure or when routine screening is appropriate.

AIDS can cause serious illness because the immune system has become severely weakened. Opportunistic infections and certain cancers can become more common or more severe. However, treatment can still improve immune function and overall health in people diagnosed with advanced HIV.

There is currently no routinely available cure that eliminates HIV from the body. ART controls the virus rather than removing it, which is why treatment generally needs to continue long term.

Effective treatment also has an important role in HIV prevention. When a person living with HIV maintains sustained viral suppression through effective ART, they do not sexually transmit HIV to their partner. This is known as Undetectable = Untransmittable (U=U).

If you are concerned about a recent HIV exposure, do not wait for symptoms to appear. If the exposure occurred within 72 hours, seek urgent medical advice about PEP. If you have ongoing concerns, HIV testing can establish your status and allow treatment to begin promptly if infection is confirmed.

For people in Singapore, HIV testing and treatment are available through various healthcare services. Early diagnosis, appropriate treatment and regular monitoring can help people living with HIV maintain their health and prevent progression to AIDS.

HIV and AIDS are not the same thing.
